SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community.
Requirements
- APRN degree
- Active and license to practice in Florida
- Hospice and Palliative Care Certification or become certified within 2 years of accepting position
Nice To Haves
- Preferred experience in the care of terminally and chronically ill patients
Responsibilities
- In-person medical assessments
- Phone consultations
- Plan of care management, including appropriate documentation and collaboration with healthcare providers and care partners
- Participation in medical staff coverage rotation
- Clinical quality assurance
- Pharmacy and utilization review
- Performance improvement activities
