About The Position

We are seeking a compassionate, skilled, and dedicated Advanced Practice Registered Nurse (APRN) to join a telemedicine palliative care team. In this specialized role, the APRN provides comprehensive, consultation-based telemedicine palliative care services to patients and families facing serious, life-limiting illnesses within the long term care setting. The role includes participation in an on-call pool providing after-hours support.

Requirements

• Hold an active, unencumbered Advanced Practice Registered Nurse (APRN) license in the State of Texas.
• Hold current national certification as a Nurse Practitioner or Clinical Nurse Specialist (e.g., AGACNP, FNP, ACGNP, or CNS).
• Have active Texas prescriptive authority and DEA registration, or be eligible to obtain them before the start date.
• Achieve a recognized Palliative Care / Advanced Certified Hospice and Palliative Nurse (ACHPN) certification within two years of joining the practice.
• Have exceptional communication skills and emotional intelligence, with the ability to navigate high-stress clinical situations.
• Participate in the on-call pool providing after-hours support.

Nice To Haves

• At least 1–2 years of dedicated experience in Hospice or Palliative Care nursing or advanced practice.
• Strong background in acute care, geriatrics, or nursing home environments.

Responsibilities

• Conduct comprehensive palliative care assessments, including detailed symptom management, via telemedicine for long term care patients.
• Lead and facilitate goals-of-care conversations and difficult end-of-life and medical decision-making discussions with clarity and empathy.
• Collaborate with patients’ facility-based primary care providers to deliver coordinated care.
• Develop and lead palliative care continuing education workshops for facility-based primary care providers.
• Participate in the on-call pool, providing after-hours support.
