About The Position

Concord Hospital is seeking a compassionate and skilled Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA-C) to join our Palliative Care team. This role provides consultative palliative care services primarily in our inpatient settings, partnering with patients, families, and interdisciplinary team members to enhance quality of life, support complex decision-making, and manage serious illness symptoms. Requirements

Master's Degree in Advanced Practice Nursing or Physician Assistant
Current New Hampshire APRN or PA license.
Current New Hampshire Registered Nurse license, for APRN role.
DEA registration.
Minimum of one year of experience in palliative care, hospice, geriatrics, oncology, critical care, family practice, or a related specialty.

Nice To Haves

Prior experience working in hospice or palliative medicine.
Strong communication and relationship-building skills.
Experience working with interdisciplinary healthcare teams.
Certification in Hospice and Palliative Care (or willingness to obtain).

Responsibilities

Provide expert palliative care consultations and symptom management for patients with serious, chronic, and life-limiting illnesses.
Conduct comprehensive patient assessments, including health histories and physical examinations.
Develop, implement, and monitor individualized care plans focused on patient goals, comfort, and quality of life.
Facilitate goals-of-care conversations, advance care planning, and discussions regarding treatment preferences.
Support patients and families through complex medical decision-making and care transitions.
Prescribe and manage medications related to palliative and hospice care needs.
Collaborate closely with physicians, nurses, social workers, chaplains, care managers, hospice teams, and community partners.
Coordinate seamless transitions across care settings, including long-term care, hospice, home care, and hospital environments.
Provide education and support to patients, families, caregivers, and healthcare professionals.
Maintain accurate, timely clinical documentation and ensure compliance with regulatory and billing requirements.
Participate in quality improvement initiatives, program development, and interdisciplinary team activities.
Discussing goals of care and advance directives.
Facilitating family meetings.
Supporting transitions from curative to comfort-focused care.
Discussing hospice eligibility and enrollment.
Addressing end-of-life decision-making and care preferences.
Providing emotional support to patients, families, and healthcare team members.
