About The Position

Physicians Alliance of Connecticut (PACT, LLC) is a multi-specialty Physician group seeking an Advanced Practice Registered Nurse (APRN) or a Physician Assistant (PA) to work in Skilled Nursing Facilities in the Milford area. This is an excellent opportunity for those that enjoy a flexible work schedule, working autonomously, and accelerate in a pay for performance environment. The Nurse Practitioner or Physician Assistant will travel to Skilled Nursing Facilities contracted with PACT, LLC. Essential Function: Provide medical care to a complex patient population in the short term rehab and long term care setting. Analyze patient’s medical history, medication allergies, physical condition and examination to determine best care for acute and chronic medical conditions. Order, interpret, and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs. Prescribe pharmaceuticals, other medications, and treatment regimens as appropriate for assessed medical conditions. Record in medical record the physical findings, formulated plans, and prognosis based on patient’s condition within appropriate time frame. Coordinate with the nursing facility care planning team in the development of comprehensive care plan. Communicate with patients and family members regarding admission, care plan, discharge needs, prognosis/end of life care as needed. Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.

Requirements

  • Masters or doctorate prepared (for APRN)
  • Certified Nationally as a Nurse Practitioner through AANP or ANCC (for APRN)
  • Possession of a valid (Connecticut) license as a Registered Nurse and an Advanced Practice Registered Nurse (for APRN)
  • Certified as a y) Nurse Practitioner by an approved certifying body (for APRN)
  • DEA, and Ct controlled substance license (for APRN)
  • Experience working with elderly patient population
  • Experience working with EHR
  • Physician Assistant Certification
  • State License (for PA)
  • Any years of experience (for PA)
  • Experience working with EHR (for PA)
