About The Position
An Internal Medical Group has opportunities for Advanced Practice Registered Nurses (APRN) or a Physician Assistants (PA) to work Part Time in Skilled Nursing Facilities in the Newington and Meriden areas. This is an excellent opportunity for those that enjoy a flexible work schedule, working autonomously, and accelerate in a pay for performance environment. The Nurse Practitioner or Physician Assistant will travel to Skilled Nursing Facilities contracted with PACT, LLC.
Requirements
- APRN: masters or doctorate prepared
- Certified Nationally as a Nurse Practitioner through AANP or ANCC
- Possession of a valid (Connecticut) license as a Registered Nurse and an Advanced Practice Registered Nurse
- Certified as a Nurse Practitioner by an approved certifying body
- DEA, and Ct controlled substance license
- Experience working with elderly patient population
- Experience working with EHR
- PA Physician Assistant Certification
- State License
- Any years of experience
- Experience working with EHR
Responsibilities
- Provide medical care to a complex patient population in the short term rehab and long term care setting
- Analyze patient’s medical history, medication allergies, physical condition and examination to determine best care for acute and chronic medical conditions
- Order, interpret, and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs
- Prescribe pharmaceuticals, other medications, and treatment regimens as appropriate for assessed medical conditions
- Record in medical record the physical findings, formulated plans, and prognosis based on patient’s condition within appropriate time frame
- Coordinate with the nursing facility care planning team in the development of comprehensive care plan
- Communicate with patients and family members regarding admission, care plan, discharge needs, prognosis/end of life care as needed
- Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards
