About The Position

OSF HealthCare Neuroscience Institute is seeking an Advanced Practice Provider (APP) to join its growing Stroke and Cerebrovascular Disease Program in Peoria, Illinois. This role offers a unique opportunity to work with a dedicated Stroke Neurologist in developing a comprehensive outpatient stroke program. The program will focus on post-hospital transitions, secondary stroke prevention, vascular risk reduction, and long-term cerebrovascular care. The APP will practice exclusively within the Hospital Outpatient Department (HOD) and be a vital part of a multidisciplinary neuroscience team dedicated to improving patient outcomes and access to care for stroke patients in Central Illinois and surrounding underserved communities.

Requirements

  • Current Illinois license as a Nurse Practitioner or Physician Assistant
  • National certification as an APRN or PA
  • Master's degree or higher in Nursing or Physician Assistant Studies
  • BLS certification required
  • Licensed to provide Advanced Practice Nurse Services or Physician Assistant Services Licensed by the Federal Drug Enforcement Administration and State of Illinois for prescribing of narcotics
  • Strong clinical assessment and communication skills
  • Ability to work independently while collaborating effectively within a multidisciplinary neuroscience team

Nice To Haves

  • Neurology, stroke, hospital medicine, emergency medicine, or critical care experience preferred

Responsibilities

  • Evaluate and manage patients following hospitalization for ischemic stroke, transient ischemic attack (TIA), intracerebral hemorrhage, cerebrovascular disease, cryptogenic stroke, small vessel disease, and carotid and intracranial vascular disease.
  • Obtain comprehensive medical histories and perform neurological examinations.
  • Monitor neurological recovery and functional outcomes.
  • Develop and implement individualized care plans.
  • Formulate differential diagnoses and treatment recommendations.
  • Review and interpret neuroimaging, laboratory studies, and diagnostic testing.
  • Coordinate specialty referrals and diagnostic evaluations.
  • Collaborate with physicians and multidisciplinary teams to optimize patient outcomes.
  • Manage modifiable vascular risk factors including hypertension, diabetes mellitus, hyperlipidemia, obesity, metabolic syndrome, tobacco use, and obstructive sleep apnea.
  • Optimize evidence-based stroke prevention therapies.
  • Monitor medication effectiveness and adherence.
  • Provide lifestyle modification counseling and risk-reduction education.
  • Facilitate smooth transitions from inpatient hospitalization to outpatient care.
  • Ensure timely post-discharge follow-up.
  • Coordinate rehabilitation, home health, therapy services, and specialty consultations.
  • Collaborate with primary care providers and community resources.
  • Support patients and families throughout the recovery process.
  • Participate in the growth and development of a comprehensive stroke program.
  • Participate in quality improvement projects, clinical innovation, and multidisciplinary program development efforts.
  • Provide comprehensive education to patients and families regarding stroke warning signs, emergency response, medication management, secondary stroke prevention, lifestyle modification, nutrition, exercise, caregiver support, and recovery expectations.
