About The Position

Provides person-centered service to assigned caseload of adult Assisted Outpatient Treatment (AOT)/HH+ "petition" or "diversion" status consumers. Services include comprehensive assessment and evaluation, care planning and coordination, outreach and crisis intervention, linkage to medical, financial, social, rehabilitation and psychiatric services, advocacy, development of social supports and procurement of needed resources. Performs recordkeeping/documentation, statistical, tracking and quality assurance activities.

Requirements

• Experience providing person-centered service to adult consumers.
• Experience with comprehensive assessment and evaluation.
• Experience with care planning and coordination.
• Experience with outreach and crisis intervention.
• Experience linking consumers to medical, financial, social, rehabilitation, and psychiatric services.
• Experience with advocacy.
• Experience developing social supports.
• Experience procuring needed resources.
• Experience with recordkeeping/documentation, statistical, tracking, and quality assurance activities.
