About The Position

Under general supervision of Antimicrobial Stewardship (AS) leadership, the ASP Pharmacist 4 is the fourth step in the four-level career ladder from Novice to Expert and is for individuals with a comprehensive understanding of antimicrobials with expertise in multiple areas of AS across diverse healthcare environments. The ASP4 works towards the system goal of improving antimicrobial utilization at the academic medical center institution and system level. The ASP4 key job functions include helping to lead, coordinate, and carry out the facility and system-wide program for the appropriate use of antimicrobials including multidisciplinary AS initiatives. They will also coordinate the agenda and minutes for assigned facility and system wide committees, and serve as the institution-wide content expert in AS in addition to the potential for expertise in particular areas of ID/AS (e.g., immunocompromised host/transplant, HIV, antimicrobial resistance). The ASP4 will organize/prioritize the education of prescribers and other healthcare personnel based on ASP1-3 feedback of interactions/intervention reports. They will review the trending adverse drug events and medication error reports provided by the ASP1-3 solutions to develop broader solution electronic medical record interventions or other modalities. Those in the ASP4 position will complete ongoing professional development in the field through self-study, mentorship and sponsored training activities, while also providing mentorship to ASP1-3, as requested by AS leadership.

Requirements

Doctoral degree in Pharmacy (PharmD) required.
License to practice as Pharmacist in Massachusetts or New Hampshire depending on work location.
Completion of PGY 2 Pharmacy Residency in Infectious Diseases plus at least 5 years’ experience, or at least 8-10 years of experience in AS and Infectious Diseases inpatient practice.
Board Certification in Infectious Diseases Practice.
Certification in Antimicrobial Stewardship from a national accrediting provider- Society of ID Pharmacists, Infectious Diseases Society of America, Society for Healthcare Epidemiology of America.
Detail oriented, critical thinker, effective communicator with analytical abilities and strong interpersonal skills.Advanced skills with computer systems required, including web-based applications and Microsoft Office suite applications.

Nice To Haves

Experience in use of electronic health record, particularly Epic, including antimicrobial stewardship tools and/or other related software preferred.
Experience in data analysis and/or data visualization; advanced degree in related field (e.g., MPH, Masters’ in Epidemiology or Statistics) a plus.
Knowledge and/or experience in related fields such as infection prevention, public health and quality improvement.
Experience in antibiogram development and management, rapid diagnostics and cascading rules in collaboration with Microbiology.

Responsibilities

Practices pharmacy with a focus on antimicrobial management in accordance with the laws, rules and regulation of the Massachusetts and New Hampshire Boards of Pharmacy, Departments of Public Health, the Food and Drug Administration, and in accordance with established policies and procedures. Maintains certification and credentials in accordance with the Board of Pharmacy and submits proof annually.
Works with infectious disease (ID) consult and primary teams on in person and telehealth initiatives. Under the direction of the AS Directors, assumes responsibility for daily team-based activities. Based on scheduling across the team, assists the Director in prioritizing and/or consolidating services and assignments of the AS team across pre-prescriptive approval reviews, post-prescriptive review and feedback, clinical decision support alert review relative to drug duplication, bug-drug mismatches, therapeutic drug monitoring and intravenous to oral conversions.
Reviews the reports from ASP3 on a weekly basis summarizing potential problem areas identified by ASP1-2 in order to prioritize targeted interventions relative to anti-infective medication orders or utilization.
Runs quality analytics on a monthly basis relative to activities of the ASP 1-3 based on interventions and provides feedback in partnership with the Director, AS.
Summarizes patient safety events on a monthly basis, responding to requests for case review, helping to propose solutions through training, computer alerts or other modalities and shares results with AS Directors and local institutional leadership, as appropriate.
Works with AS Directors to develop plans for implementation of educational programs for multidisciplinary staff at local and system level. Participates in development of prescriber and healthcare personnel education.
Serves as content expert on implementation and education related to clinical decision trees, guided decision making, EHR order questions, and antibiotic utilization and resistance tracking and reporting to the National Health Safety Network (NHSN) for specific units and the local institution. Assists with this work at the system level as directed by AS leadership.
Establishes processes to evaluate the cost effectiveness and quality of service-related clinical operations endeavors (e.g., formulary restrictions, collaborative practice models for pharmacist-driven protocols, laboratory diagnostics). Uses this information to track, analyze and provide summaries for quality metrics on adherence and outcomes.
Demonstrates good judgment and maintains strict adherence to BILH confidentiality guidelines and always maintains compliance with HIPAA.
Additional duties may be assigned based on individual skills and competencies within specific areas of AS (e.g., data analysis of dosing by indication, duration by diagnosis, diagnostic test recommendations). Support coverage of AS activities at other BILH facilities as necessary and determined by BILH AS executive leadership.
