About The Position

The AWV Navigator will work under the direction of the Population Health Manager to satisfy the EMR pre-population of upcoming Annual Wellness Visits on all qualifying patients in order to identify, address, and monitor preventative care measures for a specified patient population. Schedules patients for procedures and advises patients of surgical and financial preoperative requirements. Coordinates and schedules appointments for procedures at the clinic, ambulatory surgery center or hospital.

Requirements

  • High School Diploma, or equivalent
  • Minimum of three years of healthcare with three years of clinical experience required.
  • Excellent customer service skills.
  • Exceptional skills of independence, organization, verbal and written communication, problem-solving, professional interaction, and human relation skills, as well as analytical skills and problem-solving ability.
  • Must be PC literate with strong computer navigational skills, as well as extensive knowledge of Windows and Microsoft Office.
  • Knowledge of basic medical terminology.
  • Proficient with processes to build teams and participate in cross-functional teams.
  • Ability to work within specified timeframe requirements, including timeframes for goal achievement.
  • Must be knowledgeable of the EMR system and be able to utilize all features, including but not limited to faxing, managing CCDAs, managing order tracking, changing statuses in EMR, and adding health related information into the appropriate place in the medical record.

  • Minimum of two years of experience in Utilization Management, Case Management or Care/Disease Management preferred.

Responsibilities

  • Proactively identify, address, and track the status of quality measures and other preventative services by pre-populating Medicare Annual Wellness Visits.
  • Contact patients prior to their Annual Wellness Visit (AWV) in order to define the goals and steps of the AWV experience.
  • Conduct clinical interviews via telephone or telemedicine platform for health record inventory, health risk assessment and pre-population of the medical record in advance of the patient-provider encounter.
  • Develop proficiency and thorough understanding of evidence based clinical guidelines and HEDIS quality measures as they apply to specific patients and insurance policies.
  • Communicate and coordinate the AWV patient experience with their aligned physician and clinic staff regarding documentation completeness, preventive care needs and/or patient concerns.
  • Assist with in-person AWV health inventories, health risk assessments and chart pre- population when necessary.
  • Responsible for maintaining and managing reports detailing the patients contacted, patients not contacted, visit status, open gaps, gap status, etc.
  • Organizes and prioritizes tasks as to successfully complete duties in a timely manner.
  • Using the Practice Management system to reschedule Annual Wellness Visits as needed and in accordance with provider specific preferences.
  • Must have clear communication with your patients, team members, and manager.

  • Attend all meetings requested by the management team.
  • Have positive communication with the patients and introduce yourself to them while they are in the office if schedule allows.
  • Be knowledgeable of all Galen services to make recommendations to the patients.
