About The Position

We are seeking a compassionate and organized professional to help individuals access public benefits, community resources, and support services. This is a flexible role that offers remote work with weekly local travel to community centers and outreach events, where you will assist individuals with benefit applications and resource enrollment.

Requirements

Experience in social services, human services, case management, healthcare support, community outreach, customer service, or a related field.
Strong communication, organization, and problem-solving skills.
Ability to work independently and manage multiple priorities.
Proficiency with Microsoft Office and web-based systems.
Reliable transportation and willingness to travel locally on a weekly basis.
Reliable cell phone and internet access.
Ability to maintain confidentiality and professionalism.
Ability to work remotely while supporting community-based outreach activities.
Relevant backgrounds may include: Case Worker, Social Worker, Case Manager, Human Services Specialist, Community Outreach Coordinator, Benefits Coordinator, Patient Advocate, Community Health Worker, Family Support Specialist, or similar client-facing roles.

Responsibilities

Assist individuals in accessing benefits, services, and community resources.
Help clients complete applications, forms, and required documentation.
Attend weekly community outreach events and community centers to support individuals with benefit enrollment.
Coordinate with healthcare providers, community organizations, and service agencies.
Maintain accurate records and track case activity.
Educate clients on available programs and resources.
