About The Position

Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs. Requirements

  • Excellent communication skills including public speaking
  • Excellent interpersonal skills
  • Experience working with youth or families
  • Experience in volunteering and volunteer management
  • Foreign language speaking skills, Spanish or Mandarin
  • High school diploma or equivalency (or agree to pursue)
  • Computer literacy (PC and/or Mac), especially database and web based application
  • Qualified candidates must be able to effectively communicate with all levels of the organization.

Responsibilities

  • Patient teaching and health education
  • Track patient progress
  • Work with the healthcare team for coordination of services
  • Encourage patients participation in support groups
  • Provide patient advocacy, including interpretation, completion of paperwork, and assistance with medications
  • Help patients become more proactive and address their concerns during visits with providers
  • Anticipate and help arrange for resolution of challenges to keeping appointments (e.g., transportation, childcare)
  • Assistance with patients schedules and keeping appointments with culturally and linguistically appropriate primary care and/or specialty providers
