About The Position
The Ambulatory Telehealth RN is responsible for providing comprehensive nursing care through remote telehealth services. This position involves patient assessment, triage, care coordination, patient education, population health management, and patient navigation in UC Health ambulatory clinics using telehealth technology. The Ambulatory Telehealth RN plays an integral role in the management of patients’ care and is expected to collaborate with UC Health providers and staff to ensure high quality patient centered care.
Requirements
- Registered Nurse (RN) license
- Experience in telehealth nursing
- Proficiency in telehealth technology
- Skills in patient assessment, triage, care coordination, and education
- Ability to collaborate with healthcare providers and staff
