This is a full-time position for an Ambulatory RN in Cardiology. The role involves identifying patients needing preventive care, chronic disease management, laboratory tests, screenings, or referrals, and coordinating services to meet their needs. Responsibilities include managing INRs, stress tests, BP checks, remote device checks, and other tasks assigned by the manager. The role also encompasses transitional care management, including reviewing discharge information, medication reconciliation, and scheduling follow-up appointments. Additionally, the RN will triage patient calls, communicate test results, support medication adherence, document medical information in the EMR, and lead efforts to close preventive care gaps and monitor clinical quality measures. The goal is to promote best practices and contribute to the practice's success in achieving quality and operational goals.
