About The Position

Parkview Health is seeking a highly motivated clinical pharmacist to join the team in a unique hybrid role supporting both Parkview’s Precision Health team and the outpatient clinical oncology pharmacy services at the Parkview Packnett Family Cancer Institute (PFCI). This full-time position will be approximately 50% dedicated to Precision Health focusing in oncology and 50% dedicated to clinic-based oncology pharmacy coverage at PFCI. This pharmacist will serve in a key clinical care coordination role, connecting precision medicine, oncology providers, and patient-centered cancer care. The role will support challenging oncology cases involving tumor pathology, genomic data, molecular biomarkers, and current or proposed treatment regimens. The pharmacist will evaluate opportunities to optimize cancer therapy, improve regimen selection, support evidence-based decision-making, and utilize available tumor and genomic molecular profiles to ensure the most effective treatment option is selected, avoiding potentially ineffective therapies and thus helping to reduce the overall cost of cancer care. The Precision Health portion of this role will be highly integrated with both the Parkview Precision Health team and the PFCI cancer care teams. The pharmacist will actively participate in the newly formed PFCI Molecular Tumor Board and provide direct clinical input when tumor genetic profiling results identify opportunities for treatment optimization. This may include review of results from molecular profiling vendors such as Caris, Helix, Foundation Medicine, Natera, and others. The ambulatory oncology clinic portion of this role will provide rotating coverage across PFCI oncology clinics to support off-time coverage for existing clinic oncology specialist pharmacists. The pharmacist will work alongside hematology and medical oncology providers to assist in cancer treatment regimen selection, supportive care optimization, home medication reconciliation, and Epic Beacon Treatment Plan and Supportive Care Plan enhancement.

Requirements

• Pharmacy degree from an ACPE-accredited institution
• Completion of a PGY-2 oncology residency program or equivalent experience
• Pharmacy licensure, or eligibility, in Indiana required
• High level of experience in oncology pharmacy practice
• Strong understanding of oncology therapeutics, supportive care, and multidisciplinary cancer care
• Ability to critically evaluate clinical literature and apply evidence to patient-specific treatment decisions
• Exceptional written and verbal communication skills
• Experience participating in tumor boards, molecular tumor boards, or other multidisciplinary oncology case reviews

Nice To Haves

• Board certification in oncology pharmacy, or eligibility and intent to pursue BCOP preferred
• ACCP Applied Pharmacogenomics certification preferred and encouraged
• Experience with Epic Beacon Oncology module preferred

Responsibilities

• Review complex oncology cases involving tumor pathology, molecular profiling, genomic data, biomarker results, and current/proposed treatment strategies
• Compare genomic findings against available treatment options, current standards of care, clinical trial or expanded access opportunities, and patient-specific clinical and socioeconomic factors
• Provide evidence-based recommendations to oncology providers when therapy optimization is available based on genetic tumor profiling and ensure appropriate, payor-covered therapy use
• Actively participate in the PFCI Molecular Tumor Board, including case preparation, discussion, documentation, and follow-up recommendations
• Collaborate with Epic IS teams to optimize workflows surrounding precision medicine and tumor molecular profiling in oncology, leveraging technology to deeply integrate genetic profile results into the Epic EMR to help guide clinicians toward the best treatment options for each patient
• Support high-value cancer care by identifying opportunities to optimize therapy selection, avoid ineffective or low-value treatment, and reduce preventable cost
• Serve as a clinical resource for oncology pharmacogenomics, somatic tumor profiling, targeted therapy selection, and biomarker-driven treatment decisions
• Support development of workflows, documentation standards, clinical decision support, and educational resources related to precision oncology
• Rotate through oncology clinic areas at the PFCI to provide off-time coverage for existing hematology and solid tumor clinic oncology specialist pharmacists
• Provide at-the-elbow clinical support to hematology and medical oncology providers on cancer treatment selection and supportive care planning
• Review and optimize chemotherapy, immunotherapy, bispecific therapy, targeted therapy, and supportive care regimens
• Apply institution-specific biosimilar preference lists in direct patient care recommendations to drive financial growth by supporting cost-effective therapy selection
• Maximize patient access to therapy by collaborating with specialty pharmacy, prior authorization, financial navigation, and medication assistance teams
• Perform home medication review and assess for clinically significant drug interactions, duplicate therapy, adherence concerns, and toxicity risks
• Provide recommendations related to antiemetics, hypersensitivity prophylaxis, growth factor support, bone-modifying agents, oral anticancer therapy, anticoagulation considerations, and other oncology-related medication needs
• Aid in prior authorization denials appeals related to genetic and molecular tumor marker driven drug therapy selection
• Support Epic Beacon Treatment Plan and Supportive Care Plan optimization
• Partner with Precision Health, pathology, outpatient providers, clinic nurses, infusion pharmacists and nurses, patient navigators, dietitians, prior authorization, specialty pharmacy, research and IDS teams to deliver coordinated oncology care
