Ambulatory Nurse/Nurse Navigator - Gastroenterology - Full Time - 1st Shift
About The Position
This position is structured as two distinct role profiles, with the employee's time allocated equally between both profiles (20 hours per week for each). The Ambulatory Nurse role involves performing treatments, administering medications, assisting with procedures, providing patient/family education, and delegating tasks. The Nurse Navigator role collaborates with various staff to ensure comprehensive care plans for pediatric and adult patients in ambulatory settings, provides education to patients and parents, and supports staff and community education. This role requires a strong understanding of growth and development from infancy through adulthood. The Inflammatory Bowel Disease (IBD) Nurse Navigator specifically supports the GI Division in delivering high-quality care to patients with chronic diseases through care coordination, case management, and transitions of care, in collaboration with the Medical Director of the Inflammatory Bowel Disease Center.
Requirements
- Bachelor of Science in Nursing (BSN)
- Registered Nurse (RN) license in the state of Ohio
- 1–2 years of nursing experience
- CPR certification
- Proficient computer skills
- Strong customer service and outreach skills

- Master’s degree in Nursing
- Proficient in Microsoft Office Suite, including Word, Excel, and PowerPoint
- Experience operating and troubleshooting audiovisual (AV) equipment
Responsibilities
- Performs treatments, administers medications, and assists with procedures performed without anesthesia while utilizing aseptic technique.
- Assists with employee education and orientation and provides patient/family education with the goal of improving health outcomes.
- Involves family/significant others in planning health care and health decisions.
- Delegates and evaluates health care to assistive personnel following the hospital plan for nursing.
- Recognizes and advocates for each patient as an individual with health care needs.
- Provides considerate, respectful care affirming the patient’s right to make decisions regarding his/her medical care.
- Collaborates with medical, nursing, social services, and other staff to ensure that a comprehensive care plan is implemented for all pediatric patients and for adult patients as appropriate within the clinical ambulatory settings.
- Provides education to patients and parents in partnership with nursing staff in the designated areas.
- Supports staff and community education on the management of children and young adults across multiple departments.
- Acts as an experienced RN supporting the GI Division for the GI Department in delivering high-quality care to patients living with a chronic disease, based on assessed needs, established standards of care, and according to policy and procedures.
- Ensures the provision of quality patient care in the appropriate setting through care coordination, case management, and transitions of care to different levels of care.
- Coordinates individualized plans of care and communicates the plan to appropriate stakeholders in collaboration with the Medical Director of the Inflammatory Bowel Disease Center, the family, and the health care team.
- Ensures high-quality care is provided as efficiently and cost effectively as possible.
