About The Position

The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting. This role involves designing, implementing, assessing, monitoring, and documenting therapeutic plans, utilizing the most effective and economical medication treatments. The pharmacist will achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams. Responsibilities may include patient assessments, participating in collaborative practice agreements for medication management, ordering and interpreting medication therapy-related tests, coordinating care for wellness and disease prevention, and educating patients and caregivers. This position will also serve as a preceptor for students and residents and deliver didactic lectures. The Ambulatory Care Clinical Pharmacist will promote UVA Specialty Pharmacy’s culture of caring and communication, fostering patient empowerment through education and support. This newly created, clinic-embedded position within Rheumatology represents a strategic investment in specialty care, patient access, and relationship-based practice, designed for a pharmacist who wants to practice at the top of their license, build long-term patient relationships, and shape how specialty pharmacy services integrate into outpatient clinical care. The role is aligned with UVA Specialty Pharmacy but primarily clinic-based, partnering closely with rheumatology providers, nurses, and patients to manage complex medication therapy. The pharmacist will complement an existing rheumatology pharmacist and help expand services, capacity, and clinical impact.

Requirements

  • Bachelor's in Pharmacy or Pharm.D. from an ACPE accredited College of Pharmacy required.
  • Virginia State Board of Pharmacy License or within 60 days of hire.
  • American Heart Association (AHA) Health Care Provider BLS certification required.

  • PGY-1 residency or equivalent clinical experience preferred.

Responsibilities

  • Serve as a core clinical partner to rheumatology physicians, advanced practice providers, nursing, and clinic staff.
  • Provide longitudinal ambulatory pharmaceutical care, supporting patients over time rather than episodic encounters.
  • Design, implement, assess, monitor, and document evidence-based therapeutic plans for patients with complex rheumatologic conditions.
  • Play a meaningful role in medication access, including specialty pharmacy workflows, prior authorization support, and limited distribution of drug access.
  • Support specialty pharmacy accreditation related processes that directly impact patient access to therapy.
  • Provide patient education, counseling, and ongoing medication monitoring, fostering trust and adherence.
  • Participate in collaborative practice agreements, including ordering labs and medications when applicable.
  • Potentially support comorbid condition management or disease state care within scope and training.
  • Document care plans and clinical interventions clearly within the medical record.
  • Serve as a rotation and/or project preceptor for APPE students and residents (PGY1, PGY2 Ambulatory Care, PGY2 Ambulatory Care).
  • Deliver didactic lectures/in-services for staff and trainees.
