The Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who is a clinical expert who provides ethical, evidence-based, safe and compassionate nursing care. Provides triage, symptom management, and clinical decision support for patients with Alzheimer’s disease receiving AAT. Delivers direct outpatient nursing care, including monitoring of Anti Amyloid Targeted Therapy (ATT) infusions, recognition and escalation of adverse events, and post-infusion follow-up. Educates patients and caregivers on dementia care, ATT infusion therapy, monitoring requirements, and care plans, using communication strategies appropriate for cognitive impairment. Coordinates the continuum of care from referral or urgent/emergent follow-up through infusion treatment and ongoing outpatient management, collaborating across neurology, radiology, pharmacy, infusion services, and support teams. Documents clinical care and collects, enters, and validates required registry and outcomes data (e.g., CMS Registry) on excel spreadsheets, supporting program safety monitoring, quality improvement, and performance evaluation. Serves as a liaison among physicians, nurses, technologists, and support staff to ensure timely communication, efficient workflows, and high-quality patient care. Participates in quality, process improvement, and accreditation-related activities, including adherence to documentation standards, review of outcomes data, and implementation of standard work. Assists with daily clinic operations, staff education, orientation of new employees, and training support as needed. May complete unit/department-specific duties as outlined in department documents.
