About The Position

Hartford HealthCare Medical Group is one of the largest medical practices in New England with multiple locations throughout Connecticut and Rhode Island. We consist of a team of professionals ranging from Medical Assistants to Physician Assistants and everything in between. Our physician led medical group enjoys an excellent reputation with patients and the medical community, offering primary care, urgent care and more than 30 different specialties. The Ambulatory Care Pharmacy Specialist will collaborate as an integral member of the multidisciplinary team in the provision of care for patients in our medical clinics to optimize and promote rational, safe, evidence-based and cost-effective therapy. In addition to direct patient care responsibilities, the clinical pharmacy specialist will\: actively participate in committees and /or workgroups; collaborate in the review of new and existing medications, clinical practice guidelines and other related policies used in the management of the clinic-specific medical conditions. This person also conducts clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice.

Requirements

  • B.S or Pharm.D. Required. PharmD preferred.
  • Current CT Pharmacist license or eligibility within 45 days
  • Excellent customer service and communications skills

Nice To Haves

  • PGY- 1 Pharmacy residency preferred
  • Prior experience with EPIC systems preferred.
