The Ambulatory Care Pharmacist is responsible for providing comprehensive medication management (CMM) services to medically underserved and vulnerable populations within a Federally Qualified Health Center (FQHC). The pharmacist works as an integrated member of the Patient-Centered Medical Home (PCMH) team to improve clinical outcomes, medication access, and health equity. The pharmacist collaborates with medical providers, behavioral health, case management, and enabling services staff to optimize pharmacotherapy, address social determinates of health, and improve HRSA Uniform Data System (UDS) quality measures. Services are delivered under collaborative practice agreements and include chronic disease management, medication reconciliation, patient education, adherence support, and transitions of care.
