About The Position

This is a part-time Ambulatory Care Pharmacist position located in Tipton, IN. The schedule for this position will be a 4-day, 8-hour schedule offering flexibility to which 4 days you choose to work during the week. Provides comprehensive, value-based medication-related care to achieve definite outcomes that improve a patient's quality of life. Acts in collaboration with other healthcare professionals in the shared responsibility for patient medication therapy outcomes. Helps to manage patient chronic medical conditions and medications in between provider visits. Has the advanced knowledge and expertise to focus on the special needs of patients who may have concurrent illnesses taking multiple medications. Educates and engages patients in health promotion and wellness.

Requirements

  • Requires Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy prior to 2006.
  • Requires current Indiana Pharmacist license or eligibility for Indiana licensure as a Pharmacist.
  • Requires 0-3 years of relevant experience.

Nice To Haves

  • Successful completion of an ASHP accredited PGY1 or PGY2 residency may be required.
  • Board certification or other specialized training may be required.
  • Basic Life Support certification through the American Heart Association (AHA) may be required.
  • May require travel to IU Health facilities.
  • May require weekend rotations.
