Baptist Medical Center is seeking an Ambulatory Care Coordinator to join their team in Jacksonville, FL. This is a Full-Time opportunity working in the office, Monday through Friday, located at Baptist Metro Square. The role involves collaborating with patients and their families to facilitate discussions around advance care planning, understanding end-of-life goals and preferences, and working closely with the healthcare team to develop personalized care plans. The position requires a compassionate and patient-centered approach, providing emotional support and guidance, and maintaining accurate documentation.

Requirements

• Associate's Degree in Nursing (ASN)
• Basic Life Support (BLS)

Responsibilities

• Collaborating with patient and their families to facilitate discussions around advance care planning.
• Engage with patients to understand their end-of life goals and preferences.
• Work closely with the healthcare team to develop personalized plan of care that aligns with the patient's wishes and medical needs.
• Maintain a compassionate and patient-centered approach, providing emotional support and guidance to patients and their families during what can be a challenging time.
• Accurate documentation.
