The Ambulatory Care Clinical Pharmacist provides expertise in a specialized healthcare team and for patients to assure optimal cost-effective drug therapy. This includes developing specific pharmacotherapeutic goals, selecting drug regimens and developing a monitoring plan to meet the individualized drug therapy needs of the patient. The Pharmacist is responsible for leading efforts in the prevention and reporting of medication errors and adverse drug reactions and works to continuously improve process and practice to enhance safety and efficacy of medication therapy. This position does not directly manage other caregivers. Pharmacists may be asked to work at multiple St. Charles Health System worksites at the request of leadership to fulfill short term, immediate staffing needs.
