Hours/Shift: Full Time/Rotating Position Purpose: The DMC encompasses a team of pharmacists providing disease state medication management services through a Collaborative Practice Agreement with our community practitioners (Primary Care, Endocrinology, Oncology, Cardiology, Hepatology, and more). This is a setting where a multidisciplinary health care team approach is valued and expected. Working at the top of a pharmacist’s scope of practice, the specialist will manage a variety of disease states through delegated prescriptive and lab ordering authority. These include an emphasis on hepatitis C as part of our population health-based HCV Elimination Initiative, along with anticoagulation, diabetes with insulin pump management, hypertension, and dyslipidemia. Requirements

  • Education: Graduate of an ACPE-accredited School of Pharmacy with a Bachelor of Science in Pharmacy or Doctor of Pharmacy Degree (PharmD)
  • Experience: 3-5 years’ experience (each residency graduation counts for 3 years of experience)
  • Certification: Completion of an ASHP accredited PGY-2 Ambulatory Care Pharmacy residency and/or certification from Board of Pharmaceutical Specialties Certification in the specialty hired (i.e. BCACP), or equivalent experience providing the clinical service in ambulatory pharmacy as approved by the Director of Pharmacy.
  • License: Licensed in the State of Arizona required. New employees eligible and having applied for the AZ Board of Pharmacy will be allowed to start employment, without engaging in any Pharmacist function, until the license is granted.

Responsibilities

  • Provides clinical pharmacy services for our patients and serves as a resource, educator, mentor, and leader to physicians and other caregivers.
  • Coordinates and provides clinical pharmacy activities, including but not limited to pharmacokinetics, parenteral nutrition and general pharmaceutical care in all patient care areas as needed.
  • Identify, resolve, and prevent potential/actual medication-related problems including: Initiating, adjusting, monitoring, and recommending changes in drug therapy based on information obtained through collaborative practice; interdisciplinary rounding; patient interviews; objective monitoring parameters; review of drug profiles and patient medical records.
  • Coordinate and interpret laboratory monitoring of drug therapy, including but not limited to serum drug levels, electrolytes, and other approved drug-related laboratory testing.
  • Perform clinical pharmacy activities to decrease overall patient healthcare costs by effective pharmacotherapy, resulting in decreased length of stay, hospital readmission rates and overall better patient clinical outcomes.
  • Assist in clinical pharmacy or educational programs; serves as a mentor and preceptor in the residency training program.
  • Presents drug monographs and reviews, medication use evaluations, policy, and provides input to Pharmacy & Therapeutics Committee to support efficacious and cost-effective drug therapy.
  • Maintains interface with medical staff to provide information for new pharmaceutical supplies, formulary, drug usage, and compatibility and new regulations regarding clinical drug use.
  • Performs other duties as assigned to support overall effectiveness of department and organization.
