The Ambulatory Antimicrobial Stewardship Clinical Pharmacy Specialist will act as a role model and mentor to clinical pharmacists for clinical initiatives. This role involves aiding in the administration of medications, applying pharmacokinetic principles, and collaborating with various healthcare professionals to manage medication-related issues and ensure patient safety. The specialist will also be involved in developing educational and care plans, documenting interventions, and evaluating drug regimens for efficacy and safety. Additionally, the position requires participation in transitions of care, maintaining competence in the specialty area, and making recommendations for patient treatment goals. The role also includes providing medication-related interventions, patient-specific medication reviews, and serving as a liaison for anti-infective formulary decisions and the microbiology lab. A portion of the role involves distributive pharmacy services, including medication order entry and verification, and assisting with operational problems. The specialist will also contribute to scholarship by supporting formulary reviews, collecting data on prescribing and utilization, and publishing innovative practices. Patient experience is a key component, requiring approachable and proactive engagement. The role also emphasizes development, training, and education, including presenting at staff development programs, creating in-service education, facilitating educational activities, mentoring pharmacists, orienting new personnel, participating in performance evaluations and hiring, and precepting students and residents. Finally, the position involves significant involvement in systems of care, including adherence to drug use policies, managing medication evaluations, creating cost-saving initiatives, developing pharmacy programs, optimizing informatics, and ensuring compliance with clinical pathways. The specialist will also document medication errors, participate in strategic responses to medication shortages, and serve as a liaison between clinical teams and pharmacy leadership. Other duties as assigned include acting as a role model, adhering to policies, assisting with recalls, attending meetings, communicating effectively, and enforcing pharmaceutical care programs.
