Under the general direction of the Elder Resources Program Manager, the Aging Specialist serves as the primary point of contact for older adults, caregivers, professionals, and community members seeking information, assistance, and access to aging services and community resources. This position provides person-centered information and referral services requiring specialized knowledge of aging-related programs, benefits, and services available to older adults and caregivers. The Aging Specialist conducts brief assessments to identify needs, gathers relevant information, identifies underlying concerns, and connects individuals with appropriate resources and support that promote independence, safety, and quality of life in the community. Contacts may involve complex situations requiring problem-solving, critical thinking, strong communication skills, and sound professional judgment. The position responds to walk-in, telephone, email, and appointment requests and works directly with older adults, caregivers, and professionals to assess needs, address concerns, and identify appropriate resources and solutions. The Aging Specialist assists individuals and families in navigating complex systems and accessing programs, services, and supports that help older adults remain safely and independently in the community whenever possible. The position maintains accurate and timely documentation and collaborates with internal staff and community partners to ensure individuals receive appropriate information, assistance, and referrals. This position has no supervisory responsibilities.
