About The Position

The Aging in Place (AIP) Coordinator is responsible for assessing, coordinating and implementing holistic support services to meet the needs of elderly clients. This includes coordinating housing supports and low-barrier clinical services that foster a safe supportive environment for clients with a focus on aging in place. The AIP Coordinator will act as a subject matter expert and will work closely with a multidisciplinary team to ensure appropriate supports are provided to elderly clients.

Requirements

Bachelor’s degree in Social Work, Gerontology or a related disciplines
A minimum of three (3) years of experience working with seniors experiencing mental health, substance use health or behavioural addictions
A minimum of one (1) year of experience in supportive housing, or community mental health supports related to seniors is required
Valid drivers license and access to a reliable vehicle
Knowledge of computer software applications such as Microsoft 365 (Word, Excel, PowerPoint, Outlook, SharePoint, and client management software such as EMHware).
Knowledge of mental health, substance use health and treatment modalities.
Ability to interpret and apply residential tenancy legislation
Knowledge of community resources for individuals living with mental health, substance use health and homelessness including those specific to senior population
Knowledge of group dynamics theory and practice.
Knowledge of suicide prevention and intervention
Excellent communication, documentation, and interpersonal skills.

Eligible to work in Canada.

Bilingualism (French and English) preferred.
Training in CPR, NVCI ASIST, Psychiatric Rehabilitation, Concurrent Disorders, ASL and LSQ are assets

Responsibilities

Establish a supportive, respectful relationship with elderly clients/ tenants
Facilitate access, refer or integrate clients/ tenants to onsite supports, community supports or other community services for at risk seniors
Support elderly clients/ tenants in general domains of life: basic needs, physical and mental health, housing, financial, social, spiritual, legal, educational and vocational
Provide crisis intervention, supportive counselling and mediation when necessary
Develop appropriate timely response mechanisms to address support needs and follow up to ensure service standards are met
Plan develop and implement groups, programs, special events and services tailored to elderly clients
Adapt group delivery to meet clients at varying stages of readiness and participation
Participate in team meetings and case consultations as required
Maintain open communication with the multidisciplinary team, share knowledge and advocate to ensure elderly clients' needs are met
Maintain confidentiality, professional boundaries and provide service in accordance with applicable laws, regulations and agency policies
Provide referrals and collaborate with internal teams and external providers
Support the facilitation of access to affordable housing opportunities for seniors with behavioural addictions, substance use health and mental health challenges
Provide clinical assessment of age-related housing needs and determine preliminary suitability for housing
Support the Housing Coordinator in eviction prevention and actions with the Landlord and Tenant Board
Cooperate with the maintenance team with respect to maintenance issues as they affect tenants’ occupancy of their housing
Accompany maintenance team on property inspections and assist senior tenants with the necessary follow up
Cooperate with finance and administrative staff to ensure accurate processing and documentation of applications, tenancies and rents.
Maintain accurate, timely, confidential and ethical clinical records for each client in Salus’ client management software (EMHware) in accordance with agency policies and regulatory requirements
Participate in program evaluation and reporting on outcomes for client
Stay informed of the latest research and best practices in the fields of gerontology, mental health and substance use health
Share knowledge on aging-in-place with colleagues and members of the multidisciplinary team
Participate in regular supervision and ongoing professional development activities
