About The Position

The Child Link LPN will review the agency files for the youth in care to create a Health Report Summary outlining the youth’s health-related history, current conditions, treatments, and, as needed, instructions for the foster parents. This role involves reviewing approximately 25 files weekly, preparing detailed Health Report Summaries, ensuring accurate Medical Logs, participating in staff meetings regarding treatment goals, and arranging necessary appointments for youth. The position also requires timely and professional documentation, participation in required trainings (including CPR and First Aid), attending team meetings, and meeting with a supervisor.

Requirements

Must have valid LPN certification (completed an approved LPN training program and pass the National Council Licensure Examination for Practical Nurses).
Must be registered with IDFPR.
All educational and licensing credentials must be available and on file to meet the DCFS requirements to review the cases of children.
Meets all applicable employment regulations set by the Illinois Department of Children and Family Services and other regulatory agencies.
Must have the ability to respond appropriately to the cultural differences present among the agency's service population.
Must renew and maintain a valid RN license for the State of Illinois.

Nice To Haves

Experience with school-age children is preferred.
Ability to communicate professionally both verbally and in writing.

Responsibilities

Review approximately 25 agency files weekly for youth in care.
Prepare a Health Report Summary for each youth file reviewed, outlining treatment goals, current health status, medical treatment, therapeutic recommendations, and foster parent instructions.
Submit Health Reports to the Supervisor.
Ensure Medical Logs are filled out correctly following prescribed medication.
Participate in discussions at staff meetings regarding treatment and recovery goals and objectives for assigned youth.
Arrange for needed mental health, medical, hearing, and dental appointments for youth in the Programs.
Complete all required documentation, such as Health Report Summaries, appointment confirmation sheets, and daily sheets, in a professional manner (complete, accurate, legible, and submitted in a timely manner at the end of each day).
Participate in required trainings, including CPR and First Aid.
Attend team meetings and trainings as scheduled.
Meet with Supervisor as scheduled, coming prepared with questions, suggestions, and concerns.
Renew and maintain a valid RN license for the State of Illinois.
Perform other duties as assigned by the immediate supervisor or administrative staff.
