About The Position

The Agency RN is responsible for coordinating and monitoring healthcare services for individuals receiving support through UCP Heartland programs. This role provides direct nursing support, oversees medication and medical documentation compliance, collaborates with interdisciplinary teams, and ensures high-quality healthcare services for individuals with developmental disabilities.

Requirements

  • Active Registered Nurse (RN) license in the State of Missouri
  • Minimum of one year of experience as a PRACTICING NURSE
  • Strong medical assessment and clinical skills
  • Excellent verbal and written communication skills
  • Proficiency with basic computer systems and electronic documentation
  • Ability to work independently and within a team environment
  • Strong organizational skills and ability to meet deadlines
  • EXCEPTIONAL time management with demonstrated ability to consistently meet deadlines

Nice To Haves

  • Three or more years of experience supporting individuals with developmental disabilities

Responsibilities

  • Coordinate and oversee medical care for assigned clients
  • Conduct monthly face-to-face health assessments and monitor ongoing medical needs
  • Identify signs and symptoms requiring medical attention and respond appropriately
  • Provide nursing care and medical interventions for ill or injured clients
  • Develop individualized care plans and health risk support plans as needed
  • Participate in ISP meetings, transition meetings, and interdisciplinary care planning
  • Maintain accurate physician orders and ensure compliance with agency and DMH requirements
  • Manage and update electronic medication administration records (EMAR)
  • Review medication administration records (MARs) for accuracy and completeness
  • Coordinate medication ordering and dispensing for residential and respite settings
  • Complete all required documentation in the electronic health record system by established deadlines
  • Maintain HRST (Health Risk Screening Tool) records and assessments
  • Train, mentor, and delegate nursing-related duties to direct support staff
  • Conduct competency checks and maintain delegation records
  • Provide Level 1 Med Aide training and ongoing staff education
  • Educate staff on medication administration, infection control, and health management practices
  • Track annual physicals, immunizations, specialist appointments, and medical follow-ups
  • Complete fall risk, elopement risk, and pressure injury assessments
  • Monitor and support special dietary orders
  • Assist with funder requests, investigations, and mortality reviews
  • Ensure compliance with agency policies and healthcare regulations
  • Serve as liaison between the agency, families, physicians, pharmacies, and healthcare providers
  • Respond promptly to calls, emails, and participate in RN on-call rotation
  • Collaborate with interdisciplinary teams to support positive health outcomes
