About The Position

Anthos|Home is seeking an experienced clinical leader to serve as Aftercare Supervisor overseeing high-acuity participant stabilization. Reporting to the Vice President of Aftercare, this role oversees a team of Aftercare Specialists and Associates dedicated to serving Tier 2 and Tier 3 participants with serious mental illness, substance use disorders, or other elevated behavioral health needs. The Aftercare Supervisor will ensure high-quality, clinically informed service delivery; drive housing stability outcomes for the organization’s highest-need participants; and build and formalize the referral partnerships essential to this population’s success. Anthos|Home’s tiered stabilization model provides variable-intensity support based on participant acuity. High-acuity households are those with complex mental health, behavioral health, or functional needs requiring enhanced coordination, proactive intervention, and strong community partnerships to sustain housing stability.

Requirements

  • MSW or related graduate degree strongly preferred, with a minimum of five years of experience in behavioral health, supportive housing, high-acuity case management, or a related social services field.
  • Two or more years of supervisory experience required.
  • Demonstrated experience working with individuals with serious mental illness, substance use disorders, and/or co-occurring conditions.
  • Familiarity with ACT, Health Home, Medicaid behavioral health systems, and the NYC housing voucher landscape.
  • Strong crisis de-escalation and clinical risk assessment skills.
  • Demonstrated ability to build and sustain partnerships with external providers and community organizations.
  • Strong organizational, communication, and interpersonal skills.
  • Proficiency in MS Word, Excel, and PowerPoint; experience with HMIS or Salesforce a plus.
  • Strong data and analytical skills, including the ability to ensure accurate and timely data entry across the team, track participant outcomes and program metrics, and use data to inform supervisory decisions and drive continuous improvement.
  • Ability to travel via public transportation and ascend/descend stairs as needed, with reasonable accommodation available.

Responsibilities

  • Directly supervise up to 6 Aftercare Specialists and MSW interns carrying caseloads on the high-acuity team.
  • Provide weekly individual supervision with a focus on clinical skill development, reflective practice, and risk management.
  • Lead regular team meetings and case conferencing sessions, integrating multidisciplinary perspectives.
  • Model trauma-informed, recovery-oriented practice and support staff in applying clinical frameworks with complex populations.
  • Assist team members with escalated cases involving psychiatric crises, active substance use, or significant landlord-tenant conflict.
  • Ensure participants are assessed thoroughly and receive individualized stabilization plans that address mental health, behavioral health, housing stability, medical needs, and community integration.
  • Oversee the team’s implementation of intensive stabilization protocols, including weekly contact during the first 90 days post-placement and ongoing monitoring of clinical and behavioral risk indicators.
  • Implement evidence-based models such as Motivational Interviewing and Critical Time Intervention to advance best practices and support participant needs.
  • Support coordination of care across behavioral health, medical, and community-based providers, including ACT teams, Health Homes, and Medicaid behavioral health programs.
  • Review and guide the development and maintenance of emergency and safety plans for high-risk participants.
  • Ensure proactive identification of early destabilization markers and timely crisis prevention and response.
  • Build, formalize, and maintain a network of referral partnerships with behavioral health providers, medical providers, peer support programs, and community-based organizations serving this population.
  • Develop and maintain written partnership agreements or MOUs with key referral sources, establishing clear referral pathways, communication protocols, and shared expectations.
  • Serve as the primary organizational liaison for high-acuity service providers, representing Anthos|Home in multidisciplinary meetings, network convenings, and provider forums.
  • Identify gaps in the referral landscape and proactively pursue new partnerships to address unmet participant needs.
  • Track and report on referral activity, partnership utilization, and outcomes to inform program development.
  • Support team members in maintaining strong landlord relationships for high-acuity participants, including proactive communication and conflict de-escalation.
  • Oversee monitoring of rent payment, subsidy compliance, and housing court risk across team caseloads.
  • Assist with mediation and intervention in complex or escalated landlord-tenant situations.
  • Ensure team members maintain accurate, detailed case notes and documentation in Salesforce in accordance with program standards.
  • Monitor tier re-scoring processes at defined intervals and ensure caseload composition reflects acuity.
  • Assist with program audits and funding compliance reviews as required.
  • Participate in the on-call rotation to address after-hours participant emergencies.
  • Other duties as assigned.
