About The Position
TFI Family Services is hiring a full-time Aftercare Case Manager to join our team! This role supports children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. The Aftercare Case Manager focuses on risk management, crisis intervention, and helping families maintain stability during the aftercare period. This position involves coordinating community and educational services, maintaining accurate documentation, and meeting with families in-person to support reunification and resolve concerns.
Requirements
- A bachelor’s degree in human services.
- A valid driver’s license.
- A willingness to learn—we’ll provide all the training you need!
- Compassionate and committed to strengthening families.
- Can help families connect with the right community resources to maintain stability.
- Is an advocate for the families and children we serve.
- Eager to learn and grow in their skills to better support families and children.
Responsibilities
- Focus on risk management and help families maintain stability during the aftercare period.
- Provide crisis intervention and work with public and private agencies offering supportive services.
- Maintain accurate documentation and reports.
- Coordinate community and educational services for families.
- Meet with families in-person to support reunification and resolve any concerns.
