About The Position

We are seeking an Aftercare Case Management Specialist to join our team. Your role will use advanced knowledge in the area of child welfare. You will exercise discretion and independent judgment in making decisions on the overall delivery of services to children and families on their caseload. You will coordinate and partner with the family, the family’s natural support and members of the assigned team to safely maintain children in their homes and assist the family with the natural adjustment that comes following reintegration. As a member of the KS Foster Care team, you will work with other team members and report to our Manager of Aftercare.

Requirements

  • At least 3 years of experience with children and families
  • Bachelor’s degree in social work or other human services related field.
  • At least 21 years of age
  • Pass background check, physical, and drug screening
  • A valid driver’s license in the state you reside in, proof of current vehicle insurance, and reliable transportation.

Nice To Haves

  • Master's degree in social work or other human services related field and licensed by the Behavioral Sciences Regulatory Board to practice in Kansas are preferred.

Responsibilities

  • Manage and provide services to children and their families following their return to the home of removal.
  • Continually analyze and assess each family and child situation on an individual basis, using advanced knowledge in the area of child welfare reintegration and adoptions to develop recommendations regarding supportive services and resources that each child and family may need.
  • Facilitate service planning with the child/youth, family and other members of the case team in accordance with Cornerstones of Care and the Kansas Department for Children and Families policy (DCF).
  • For court involved cases, prepare and submit high quality reports providing updates on child and family progress within required time frames.
  • Be accessible via cell phone to children, families, placement providers, DCF staff, and other Cornerstones of Care team members in accordance with on-call policy.
