About The Position

Elara Caring is seeking a compassionate After-Hours/On Call Registered Nurse to provide hospice care in Springfield, Illinois. The nurse assesses hospice patients and their families, monitors patient conditions, communicates significant changes and safety concerns to physicians and the care team, and supports implementation of interdisciplinary Plans of Care. The role also includes educating patients, families, caregivers, and facility staff about hospice philosophy, pain management, and symptom management, as well as providing compassionate support at the time of death and completing bereavement assessments.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited certificate, diploma, associate, or baccalaureate school of nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred but not required.
• Experience in a hospice or home health environment is preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
