About The Position

The After-Hours RN is responsible for providing overnight triage, support, and in-person care to hospice patients and families. This role focuses on managing urgent needs, providing symptom management, and supporting patients at end-of-life during overnight hours. This position is designed to reduce the overnight burden on daytime clinical staff while ensuring timely, compassionate care for patients and families.

Requirements

• Active, unencumbered RN license in Colorado
• Minimum 2 years of nursing experience required
• Hospice or Home Health experience required
• Comfortable working independently overnight
• Strong assessment, triage, and critical thinking skills
• Excellent communication and documentation skills
• Ability to respond to calls and travel within service area as needed

Responsibilities

• Respond to after-hours calls from patients, families, and facilities
• Provide clinical triage and guidance over the phone
• Make in-person visits as needed for symptom management, imminent death support, and urgent clinical needs
• Perform death visits, including pronouncement and support for families
• Communicate clearly with providers and interdisciplinary team as needed
• Document all patient interactions in a timely and accurate manner
• Ensure continuity of care by providing updates to the daytime team
• Provide emotional support to patients and families during overnight hours
