About The Position
Hospice Nurse After-hours LPN Salary: $31.25-$34.13 BOE Schedule: Full time |Mon - Thur 8P-8A Location: Blue Ash, Ohio, and the surrounding area Requirements of the Hospice Nurse role: Licensed Practical Nurse (LPN) in the state of practice A minimum of 1 year of acute care experience Hospice experience preferred Valid driver's License and Auto insurance coverage Reliable transportation: Willing to travel About the Hospice Nurse ro le: Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients. Provide on-call services after hours, and complete skilled visits. Assist RNCM in carrying out the plan of care. Assist the Nursing team in performing specialized procedures( as defined in the state nurse practice act) as directed by the RNCM under the physician's orders. Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed. Assist in the reinforcement of patient and family education regarding end-of-life care. Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning. Keep the RNCM informed of changes noted during the process. Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research and reading. Requirements
