About The Position
Provide advocacy, support, and case management services to members of the Homeless Families Program.
Requirements
- Two to four years college, post-high school training or relevant work experience required.
- At least two years of experience in case management or related position preferred.
- Advocacy and outreach skills required.
Nice To Haves
- BS/BA strongly preferred.
Responsibilities
- Assesses clients’ needs and assists with information and/or referral to community resources, such as legal services, educational opportunities and medical care.
- Provides immediate response to social service emergencies.
- Uses initial and follow-up assessments and case management assessments to identify trauma, mental health, dental and medical visits.
- Develops close working relationships with community resources that provide services to homeless parents and children including homeless shelters.
- Maintains client records including assessments, mental health and medical and home visits.
- Any and all other duties as assigned.
