About The Position

As a Homeless Services Case Manager at SJCH, you will play a vital role in providing comprehensive support and resources to individuals experiencing homelessness. You will work closely with clients in assessing their needs, developing personalized action plans, and connecting them with appropriate services and resources to help them achieve housing stability. With your expertise and empathy, you will have a direct impact on improving the lives of those facing homelessness.

Requirements

  • Bachelor's degree in social work, human services, psychology, or a related field (Master's degree preferred).
  • Proven experience in case management or social work, preferably in homeless services or related fields.
  • Knowledge of local resources and services available for individuals experiencing homelessness.
  • Strong interpersonal and communication skills, with the ability to build rapport and establish trusting relationships with clients from diverse backgrounds.
  • Excellent organizational and time management skills, with the ability to prioritize tasks and manage a caseload effectively.
  • Empathy, compassion, and sensitivity in working with individuals facing homelessness and the challenges they may encounter.
  • Proficient in documentation and record-keeping, ensuring accuracy and compliance with confidentiality standards.
  • Ability to work both independently and collaboratively within a multidisciplinary team.
  • Familiarity with relevant regulations and guidelines, such as HUD guidelines for homeless services (preferred).
  • Valid driver's license and reliable transportation (if required for home visits or off-site meetings).
  • Certified Application Assistant (CAA Certificate)
  • Bilingual English/Spanish (Required)

Responsibilities

  • Conduct thorough assessments to identify the specific needs and goals of clients experiencing homelessness.
  • Collaborate with clients to develop personalized action plans that address their unique circumstances and facilitate their journey towards achieving housing stability.
  • Provide crisis intervention and support to clients, offering immediate assistance and referral to appropriate resources as needed.
  • Assist clients in accessing and navigating available housing programs, benefits, and services, such as emergency shelters, transitional housing, and supportive housing programs.
  • Conduct regular check-ins and follow-up with clients to monitor their progress and adjust their action plans as necessary.
  • Leverage community resources and partnerships to connect clients with vital services, including mental health and substance abuse treatment, healthcare, employment and vocational training, education, and other support services.
  • Advocate on behalf of clients to ensure they receive appropriate care and assistance from partnering agencies and organizations.
  • Maintain detailed and accurate client records, documenting all interactions, assessments, action plans, and progress notes in compliance with confidentiality and regulatory requirements.
  • Collaborate and coordinate with a multidisciplinary team of professionals, such as social workers, housing coordinators, healthcare providers, and community organizations, to provide comprehensive care and support to clients.
  • Stay informed about current trends and best practices in homeless services and related fields, continuously enhancing your knowledge and skills through professional development opportunities.
