Assesses the physical and psychosocial status of patient's by means of interview health history physical examination and diagnostic studies. Recognizes deviations from normal and physical assessment and formulates treatment plans for health problems and follow-up. Prescribes medications and transfusions based on laboratory results, routine diagnostic studies, and treatment care plan. Reviews patient chart and determines eligibility for clinical trial. Collaborates with other members of the healthcare team and requests consultation from other healthcare professionals as appropriate. Interprets and evaluates findings from diagnostic studies. Relays appropriate information regarding patient care to the collaborating or supervising physician. Administers therapeutic measures and obtain specimens as appropriate. Evaluates the quality of care provided and recommends changes for improvement in the delivery of care. Initiates appropriate actions to facilitate the implementation of therapeutic plans consistent with the continuing care needs of patients. Documents history & physical, treatment care plans, daily progress notes, discharge summaries, and provider follow-up letters. Assists in the management of family dynamics and coping mechanisms during the acute and chronic phase of the patient's care. Performs procedures and assists in the operating room in accordance with medical staff privileges. Triages patients appropriately and provides emergency treatment is necessary. Considers the physical, emotional, socioeconomic status, cultural, and health literacy barriers for patients, families, and communities when providing care. Ensures that departmental and interdepartmental policies, procedures, and standards of care are maintained. Assists in interpreting these policies for patients, visitors, and staff. Informs supervisors, administrators, and executives of actual and potential problem that require resolution at the administrative level. Serves on departmental or administrative committees as requested. Educates patients and families about the disease process and treatment. Provides information to patients and families related to symptom management. Participates in the development of clinical pathways and educational materials. Identifies, analyzes, and resolves patient care problems, resulting in system improvements or improvements in patient care. Participates in continuous quality improvement activities. Demonstrates a commitment to continue improving the quality of care provided to patients. Applies current concepts and principles from research to the practice setting. Implements changes in clinical practice based on findings from research and quality improvement projects. Participates in multidisciplinary clinical research activities, clinical trials, and acts a patient advocate. May perform all duties in a mobile environment including travel and overnight stays. Conducts oneself in accordance with established organizational and departmental policies and procedures, objectives, quality assurance programs, safety and compliance standards. Conducts oneself in accordance with standards of professionalism in all aspects of work interactions and communications. Conducts oneself in accordance with organizational and departmental attendance standards. Enhances professional growth and development by participating in educational programs, reading current literature, and participating in in-service meetings and workshops. Prepares written documentation as required by the profession and the department; maintains, distributes, analyzes and projects information for required records, reports and statistics as directed. Shares expertise with colleagues, offers guidance and ensures knowledge transfer, as needed and as appropriate to the role. Attends meetings as required. Performs other related duties as assigned or requested.
