About The Position

Ohio’s Hospice is seeking a compassionate and experienced Advanced Practice Registered Nurse (APRN) to provide expert, patient-centered care to individuals living with serious and life-limiting illnesses, as well as their families. The APRN provides advanced clinical assessment, diagnosis, symptom management, and care coordination while developing individualized plans of care tailored to each patient’s goals and needs. Working collaboratively with patients, families, caregivers, physicians, and the interdisciplinary care team, the APRN helps promote comfort, maintain or improve functional status, facilitate effective communication, and support the highest possible quality of life. Requirements

  • Master of Science in Nursing (MSN).
  • Minimum of 2 years of clinical experience as an APRN.
  • Active, unrestricted Registered Nurse (RN) license in the State of Ohio, with no current board

Responsibilities

  • Provide advanced clinical care to patients with serious and life-limiting illnesses.
  • Conduct comprehensive patient assessments and synthesize complex clinical information.
  • Evaluate and diagnose patient conditions within the scope of APRN practice.
  • Develop, implement, and coordinate individualized plans of care.
  • Assess and manage pain and other symptoms to promote comfort and quality of life.
  • Monitor patient progress and modify treatment plans as clinically appropriate.
  • Collaborate with physicians and interdisciplinary team members to provide coordinated, patient-centered care.
  • Communicate effectively with patients, families, caregivers, and healthcare providers regarding goals of care, treatment options, and changes in patient condition.
  • Provide education, guidance, and emotional support to patients and families throughout their hospice and palliative care journey.
  • Maintain accurate, timely, and comprehensive documentation of assessments, interventions, and plans of care in the electronic medical record.
  • Serve as a clinical resource and advocate for patients and their families.
