About The Position

The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity. Nurse Practitioner Focuses on providing comprehensive primary and geriatric care to older adults so they can remain safely and independently in their homes. Working as part of an interdisciplinary team, the Nurse Practitioner assesses, diagnoses, and manages acute and chronic health conditions while developing individualized care plans and coordinating services with other healthcare professionals. The role emphasizes preventive care, chronic disease management, and reducing hospitalizations through ongoing monitoring and participant-centered support. Nurse Practitioners may provide care in clinic, home, or community settings and work closely with participants and families to improve overall health, safety, and quality of life.

Requirements

  • Graduate of an accredited Nurse Practitioner program
  • Current APRN licensure and board certification
  • Prescriptive authority in the state of Kansas
  • Experience in primary care, geriatrics, or chronic disease management
  • Strong assessment, communication, and care coordination skills
  • Ability to work collaboratively within an interdisciplinary team
  • CPR certification
  • Reliable transportation and ability to travel for home/community visits
  • Ability to work in clinic, home, and community-based settings

Nice To Haves

  • Previous experience in a PACE program
  • Background in home health, hospice, long-term care, or community-based care
  • Experience working with older adult populations and complex medical conditions
  • Familiarity with electronic health records (EHR)
  • Strong participant-centered and preventive care approach
  • Ability to work independently and manage changing participant needs
  • Experience collaborating with families and caregivers
  • Ability to recognize early changes in condition to help prevent hospitalizations
