About The Position

The Advanced Practice Provider provides comprehensive care for common acute and chronic health conditions while collaborating with a multidisciplinary care team. This role supports the Patient-Centered Medical Home model through direct patient care, coordination, consultation, education, and referrals.

Requirements

Graduation from Accredited APRN Certification Program
Active/Clear Florida APRN License
Board Certified
This position requires a Level 2 background screening through the Florida Care Provider Background Screening Clearinghouse.

Responsibilities

Provide comprehensive primary care focused on health promotion, illness prevention, and health restoration for patients across the lifespan.
Conduct thorough patient histories and physical examinations, assess health concerns, and develop appropriate medical and/or nursing diagnoses.
Order, perform, and interpret appropriate diagnostic evaluations, including laboratory and radiographic testing, and coordinate consultations with specialists and other healthcare providers as needed.
Develop individualized treatment plans based on each patient’s needs, including prescribing medications, providing appropriate interventions, and establishing follow-up care.
Monitor patient progress and coordinate ongoing care to ensure patients receive timely, appropriate, and comprehensive services.
Provide education and guidance to patients and families on health maintenance, disease prevention, healthy lifestyle choices, and management of acute and chronic conditions.
Collaborate closely with physicians, nurses, and other members of the care team to support coordinated, patient-centered care and actively participate in daily team huddles.
Serve as a clinical resource by providing consultation and support to patients, families, physicians, care team members, and nursing staff on health maintenance, referrals, and patient care needs.
Participate in quality assurance, patient care, and program review meetings, as designated by the Medical Director, and complete continuing education required to maintain APRN licensure and clinical competency.
Mentor and serve as a preceptor for Physician Assistant, Nursing, and APRN students while supporting the development of future healthcare professionals.
