The Advanced Practice Provider (APP) is a primary care provider responsible for managing uncomplicated acute and stable chronic illnesses. The APP performs health assessments, early illness detection, health promotion, disease prevention, and management. They may also develop and coordinate special programs within their area of expertise. This role involves comprehensive patient care, including diagnosis, treatment planning, medication prescription, patient education, and collaboration with other healthcare professionals. The APP is also responsible for timely medical record documentation and billing, and may delegate tasks to clinical staff. Care may be provided at offsite locations, and on-call responsibilities are expected. Maintaining current knowledge and professional development is essential, as is accessing resources for underserved patients. Participation in provider staff meetings and Continuous Quality Improvement committees is also required.
