Responsible for the provision of primary care medical services in a Patient-Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations. The provider will conduct examinations, review laboratory, referral, and imaging results to determine diagnoses, and accurately document visits using appropriate templates. The position requires accurate record-keeping to attain a diagnosis, meeting budgeted visits and clinical care measures, and locking records within the 72-hour window. The role also involves meeting Advanced Practice Provider professional focus and goals, providing comprehensive, continuous, and coordinated medical care for acute and chronic illnesses across all ages within a family unit in a community setting. This includes health promotion, maintenance, illness prevention, and applying a holistic healthcare model that addresses both protective and risk factors. Management of chronic diseases and acute illnesses requires a broad range of knowledge in procedural skills, coordination with specialists, and utilization of community resources. The provider will advocate for access to healthcare, focusing on structured evaluation, early intervention, and health promotion. Consultation with supervising physicians or team members is expected for complex cases. The role actively promotes and works within the Patient-Centered Health Home model of care, ensuring excellent customer service and courteous interactions with patients, families, and staff. Awareness of wait times and providing patients with appropriate educational materials based on health literacy needs are key responsibilities. The provider will be accountable for patient status as measured by clinical quality measures, promote and participate in daily huddles, and incorporate ancillary health team members in patient care. Setting and monitoring self-management plans, assisting in the assurance of clinical procedures, and maintaining up-to-date clinical protocols are also required. Chart and peer review responsibilities must be completed in a timely manner. The provider will assist the patient care team with performance improvement efforts, staff education, and FHCN promotion, as well as performance evaluations and quality improvement. Participation in the rotation of medical, physician assistant students, and dental residents is relevant. The role includes participation in Network Health Fairs and other community events, as well as meaningful participation in site and provider meetings. Meeting Network and credentialing expectations for licensure and employment is mandatory. Adherence to the Attendance and Absenteeism Policy is critical, recognizing that regular attendance is an essential function. The ability to present to and work at any FHCN location, based on business need, is required. Other duties as assigned.
