About The Position

The Advanced Practice Provider (APP) may be a Nurse Practitioner or Physician Assistant. The advanced practice provider is responsible for the assessment, medical case management, diagnosis, treatment, follow-up and referral of clinic patients seen at the North Fork Office, Quincy Office or Indian Valley Office. The APP plans patient care using skills specific to his/her individual training and experience and works under established protocols. We are located at 1065 Bucks Lake Road, Quincy, CA 95971.

Requirements

  • Must be a graduate of a nurse practitioner program that meets the standards of the Board of Registered Nursing (BRN) in California or a graduate from a physician assistant program that meets the standards of the Physician Assistant Committee of the Medical Board of California.
  • Must be licensed in California.
  • Current CPR certification.
  • Graduation from a nurse practitioner program which meets the standards of the California BRN or physician assistant program that meets the standards of the Physician Assistant Committee of the medical Board of California and completion of training consistent with the procedures to be performed.
  • Excellent interpersonal skills with patients required.
  • Ability to communicate effectively with team members, patients and community members.
  • Must demonstrate responsibility for keeping credentials current and for updating his/her knowledge.
  • Must demonstrate reliable and punctual attendance.
  • Must have a friendly and helpful disposition toward visitors and staff.

Responsibilities

  • Demonstrates knowledge and ability to gather data pertinent to patient plan of care / assessment.
  • Performs appropriate patient intake related to compliant, medical, social and family history.
  • Utilizes patient chart, hospital chart or emergency room records to gather data when needed.
  • Performs appropriate physical assessments.
  • Has ability to complete thorough physical exam on male and female patients commensurate with licensure and to understand normal variations.
  • Demonstrates knowledge of normal growth and development and how different age groups affect patient ability to understand and utilize information in a meaningful way
  • Establishes a plan of care which will assist the patient to move toward better health and healing, that is comprehensive and problem specific based upon established protocols or standards of care
  • Performs medication reconciliation and gathers information on adverse drug reactions, allergies and idiosyncratic reactions.

Our program is operational from 7:45am to 5:15pm, Monday through Friday, and is approved for children aged 0 to 5 years.
