Responsible for the provision of primary care medical services in a Patient-Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations. The provider will conduct examinations, review laboratory, referral, and imaging results to determine diagnoses, and document visits accurately using appropriate templates. Key accountabilities include attaining budgeted visits, meeting clinical care measures, and locking records within the 72-hour window. The position also requires meeting Advanced Practice Provider professional focus and goals, providing comprehensive, continuous, and coordinated medical care for acute and chronic illnesses across all ages within a family unit in a community setting. This includes health promotion, illness prevention, and applying a holistic healthcare model. Management of chronic diseases and acute illness, coordination with specialists, and utilization of community resources are essential. The role advocates for access to healthcare, focusing on structured evaluation, early intervention, and health promotion, and consults with supervising physicians or team members when necessary. The provider will work within and promote the Patient-Centered Health Home model of care, provide excellent customer service, and ensure patients receive appropriate educational materials. Accountability for patient status is measured by clinical quality measures, and the provider will promote and participate in daily huddles, incorporate ancillary health team members, set and monitor self-management plans, and assist in assuring clinical procedures and protocols are up-to-date. Chart and peer review responsibilities must be completed in a timely manner. The role also assists the patient care team with performance improvement, staff education, and FHCN promotion, potentially participating in the rotation of medical, physician assistant students, and dental residents. Participation in Network Health Fairs and community events, as well as site and provider meetings, is expected. The provider must meet Network and credentialing expectations for licensure and employment, adhere to the Attendance and Absenteeism Policy, and be able to present to and work at any FHCN location based on business need.
