About The Position

Responsible for the provision of primary care medical services in a Patient-Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations. The provider will conduct examinations, review laboratory, referral, and imaging results to determine diagnoses, and document visits accurately using appropriate templates. Key accountabilities include attaining budgeted visits, meeting clinical care measures, and locking records within the 72-hour window. The position also requires meeting Advanced Practice Provider professional focus and goals, providing comprehensive, continuous, and coordinated medical care for acute and chronic illnesses across all ages within a family unit in a community setting. This includes health promotion, illness prevention, and applying a holistic healthcare model. Management of chronic diseases and acute illness, coordination with specialists, and utilization of community resources are essential. The role advocates for access to healthcare, focusing on structured evaluation, early intervention, and health promotion, and consults with supervising physicians or team members when necessary. The provider will work within and promote the Patient-Centered Health Home model of care, provide excellent customer service, and ensure patients receive appropriate educational materials. Accountability for patient status is measured by clinical quality measures, and the provider will promote and participate in daily huddles, incorporate ancillary health team members, set and monitor self-management plans, and assist in assuring clinical procedures and protocols are up-to-date. Chart and peer review responsibilities must be completed in a timely manner. The role also assists the patient care team with performance improvement, staff education, and FHCN promotion, potentially participating in the rotation of medical, physician assistant students, and dental residents. Participation in Network Health Fairs and community events, as well as site and provider meetings, is expected. The provider must meet Network and credentialing expectations for licensure and employment, adhere to the Attendance and Absenteeism Policy, and be able to present to and work at any FHCN location based on business need. Requirements

Possesses specific advanced knowledge and skills, including written and verbal communication skills, computational, computer, and technical skills, and mathematical knowledge.
Completion of an accredited Nurse Practitioner or Physician Assistant training program, with a minimum cumulative GPA of 2.5.
Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.
Equivalent of extensive formal training as a Nurse Practitioner or Physician Assistant, including an understanding of the application of the theory and practices of the profession to the operation of the organization as part of a Master’s degree (or other applicable extended training program).
Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions, and other elements.
Licensed in the state of California as a Nurse Practitioner and/or licensed in the state of California as a Physician Assistant.
Effectively communicate professional opinions and extrapolations of information collected, synthesized/analyzed.
Determine appropriate methods of communicating information through the use of tables, graphs, charts, and other visual forms.
Prepare and execute presentations to large groups.
Sit and use repetitive hand movements to type and grasp.
Frequently stand or walk.
Occasionally lift and/or move up to 20 pounds.

Provide comprehensive health care including health promotion and maintenance, prevention of illness and disability and application of holistic healthcare model with focus on addressing both protective factors and risk factors affecting the health and wellbeing of patient.
Manage chronic diseases and acute illness with a broad range of knowledge in procedural skills, coordination of care with specialists, and utilization of community resources when appropriate.
Advocate access to health care with a focus on structured evaluation, early intervention, and health promotion.
Consult with supervising physician or other team members on cases that need further input.
Work within and help to promote the Patient-Centered Health Home model of care.
Provide excellent customer service and care.
Consistently behave courteously when interacting with patients/ family members, support staff, and Network staff.
Remain aware of wait time during the daily workflow.
Provide the patient with educational materials and resources appropriate to the patient’s health literacy needs.
Accountable for patient status as measured by relevant clinical quality measures.
Promote and participate in daily huddles.
Incorporate the participation of ancillary health team members in managing the care of patients/families.
Set and monitor self-management plans.
Assist in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
Complete chart and peer review responsibility in a timely manner.
Assist the patient care team with performance improvement efforts, staff education, and FHCN promotion.
Assist the patient care team with performance evaluations and quality improvement.
Participate in rotation of medical, physician assistant students and dental residents where relevant.
Participate in Network Health Fairs and other community events.
Participate in site and provider meetings in a meaningful way.
Meet Network and credentialing expectations for licensure and employment.
Adhere to the Attendance and Absenteeism Policy, recognizing that regular attendance is considered an essential function of all FHCN positions.
Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.
Perform other duties as assigned.
