The FHCN PACE Advanced Practice Provider has the responsibility for providing direct and indirect care to the participants of the PACE Program in the context of the Interdisciplinary Team (IDT). This role involves performing comprehensive history and physicals, evaluating and treating episodic illnesses, coordinating care in inpatient and nursing home settings, and actively participating in family conferences and care planning. The provider will also coordinate 24-hour care delivery, provide after-hours call coverage, document participant changes, and maintain education regarding geriatric population best practices. Participation in committees and quality improvement processes is expected, along with encouraging Advanced Directives and discussing DNR options. The role requires meeting credentialing expectations, adhering to attendance policies, and the ability to work at any FHCN location as needed.
