About The Position

Lykos Medical, part of Brigade Health and its Affiliates, is seeking a Physician Assistant or Nurse Practitioner for its Mobile Primary Care Team. The clinician provides care to seniors in their homes and communities, managing an independent patient panel while working with a collaborative team and clinical and operational leaders. The role emphasizes personalized, relationship-centered care, clinical excellence, autonomy, accountability, and an entrepreneurial approach to building community relationships and expanding access to healthcare. The position may be part-time or full-time, depending on the provider’s goals. The posted hourly range is $65–$90 USD.

Requirements

• Active, unrestricted Nurse Practitioner or Physician Assistant license in the state of practice.
• Minimum 1–2 years of clinical experience.
• Strong clinical judgment and ability to work independently.
• Professional, dependable, and trustworthy approach to patient care.
• Excellent relationship-building and communication skills.
• Entrepreneurial mindset and passion for developing provider-community relationships.
• Adaptability, accountability, and commitment to exceptional patient experiences.
• Valid driver’s license and reliable transportation.

Nice To Haves

• Experience in geriatrics, primary care, home-based care, or value-based care is preferred.
