About The Position

Our section prides itself on its strong and integrated programs in clinical care, research, and education. The strength of our clinical programs is essential not only to our ability to provide excellent patient care, but to our ability to perform research that advances the frontiers of clinical practice and to train future generations of physicians and advanced practice providers in hospital medicine. Our research programs allow us to advance the scientific basis and clinical practice of hospital medicine and to train hospital medicine researchers who can carry this scientific mission into the future, often finding inspiration in the desire to solve clinical dilemmas we encounter in our own practices. Likewise, our training programs allow us to bring talented young persons into the relatively new field of hospital medicine, preparing them for productive and sustainable careers as clinicians, researchers, and educators. Our APPs are a vital component of our section. They work at the highest scope of practice clinically, educate advanced practice provider students, and have opportunities for robust mentorship to pursue passions within the academic realm. The Advanced Practice Provider (APP) will provide specialized care to patients on the inpatient Hospital Medicine service inclusive of general medicine, transplant co-management, admission services and hospitalist cardiology and oncology for the Section of Hospital Medicine in the Department of Medicine. In this role, the APP will utilize advanced assessment skills and knowledge of clinical therapeutics to evaluate, diagnose and manage patients on the Hospitalist Services. The APP orders and interprets appropriate diagnostic tests, therapeutic interventions and prescribes pharmacological agents as necessary from admission to discharge. As an APP, they will maintain continuity of patient care through patient follow-up, monitoring of patient progress and modifying plan of care as necessary and per scheduling requirements. The APP applies knowledge of new technology and resources as per competencies obtained through the mentorship/collaboration of the hospitalist attendings demonstrating advanced or specialized expertise. This role will be a part of a team of APPs working in the Hospitalist space who provide complete care to the hospitalized patient. This role will train and function with our team at the Hyde Park campus.

Requirements

  • A college or university degree from an accredited school/college.
  • Knowledge and skills developed through a combination of education and a minimum of 2 years of relevant work experience in a related job discipline.
  • Current and/or eligible for State of Illinois licensure/registration.
  • Eligibility to be privileged and credentialed through the Medical Staff Office (MSO).
  • Conduct physical examinations and health assessments.
  • Interview and obtain medical information.
  • Proficiency in the use of computers.

  • Once licensed and as needed, apply for a mid-level practitioner controlled substance license and DEA number.
  • Privileged and credentialed through UCM Medical Staff Office prior to start date.

Responsibilities

  • Obtain and complete history for daytime admissions to Hospital Medicine In- patient services.
  • Round on APP Hospital Medicine Services completing full spectrum of care coordination.
  • Perform and document physical examinations according to government guidelines.
  • Prepare admission, rounding, and discharge orders in collaboration of the attending physician for patients on the inpatient services.
  • Perform daily assessment and physical examination prior to multidisciplinary rounds.
  • Perform additional physical assessments of patients during the day as clinically indicated.
  • Order appropriate laboratory tests according to clinical diagnosis, clinical pathways and protocols.
  • Arrange and order appropriate diagnostic studies for patients under the collaboration of the attending physician.
  • Monitor and discuss diagnostic studies and results of laboratory tests with attending physicians, consultants, patients, and patient families.
  • Arrange appropriate inpatient consult services in collaboration of the attending physician.
  • As part of duties, perform reporting for government agencies, chart review, facilitate prescription medications at discharge and respond to patient and family inquiries as appropriate.
  • Coordinate discharge planning with multidisciplinary team.
  • Under physician supervision, directs and coordinates care of hospitalist patients.
  • The clinician will complete detailed histories and physicals collect and interpret reports from diagnostic lab tests, radiographic procedures, and clinical studies; as well as order tests, prescribe medications.
  • Differentiates normal and abnormal variations for all body systems.
  • Accurately interprets diagnostic tests.
  • Gathers pertinent information systematically and skillfully from all sources: history, diagnostic tests, comprehensive physical examination, and psycho-social assessments.
  • Identifies educational needs of the patient and family and assists with teaching.
  • Incorporates assessment findings into a plan to direct patient care.
  • Accurately and appropriately performs routine diagnostic and therapeutic techniques according to established protocols and standardized procedures for advanced practice providers.
  • Initiates referrals based on patient and family need.
  • Maintains formal and informal systems for information sharing, planning and problem solving with clinic staff, physician partners and other Advanced Practice Providers.
  • Analyzes multiple sources of data in making clinical judgments and in determining the effectiveness of a plan of care.
  • Participates in research projects in the practice area.
  • Performs other related duties and initiatives as deemed necessary in the best interests of patient care.
  • Performs bedside evaluation and triage need for consultants and attendings to come to bedside.
  • Participates in the clinical onboarding of new team members as the team sees fit.
  • Evaluates goals and outcomes taking into account the benefit-versus-burden, safety, quality, and cost-effectiveness for the patient, family and the organization.
  • Develops diagnostic strategies and therapeutic interventions needed to achieve the goals and outcomes of the patient’s plan of care.
  • Performs other related work as needed.
