About The Position

The Advance Practice Provider (APP) shall provide excellent customer service. The APP works collaboratively and professionally with the other clinical staff, under the direct supervision of the Medical Director, for the effective and efficient delivery of medical care services. The duties of the APP shall not exceed those duties permitted by Utah State Laws. The APP is required to perform duties as outlined in this job description.

Requirements

  • Successful completion of an approved nurse practitioner or physician assistant program.
  • State accreditation based on certifying exam.
  • Basic Life Support Certificate required upon hire.
  • Ability to work well as a team, exhibit integrity, use good judgment and keep confidential information.
  • Demonstrate reliable attendance and punctuality consistently.
  • Demonstrated competencies in dealing with all age groups including neonates, infants, children, adolescents, adults, and geriatric patients.
  • Influenza, Tdap, Hep B, MMR, Varicella, TB test, and Negative Drug Test results.
  • Ability to stand, bend at the waist, kneel, reach over the head, talk, hear, and see.
  • Must be able to move or lift documents and materials weighing up to 30 pounds.

  • Bilingual in English and Spanish strongly preferred.
  • Additional Vaccines may be required.

Responsibilities

  • Conducts home visits to deliver patient-centered care and treatment, working collaboratively with a Medical Assistant to perform assessments, interventions, and follow-up in a home setting.
  • Conducts patient assessments including medical history, physical exams, and documentation of findings; establishes preliminary diagnoses and determines appropriate follow-up care.
  • Orders, administers, and interprets diagnostic tests such as X-rays, electrocardiograms, laboratory tests, and other screenings.
  • Provides treatment for acute and chronic conditions, including minor injuries, through therapeutic procedures such as immunizations, injections, suturing, wound care, and infection management.
  • Develops, implements, and monitors patient care plans; educates and counsels patients on treatment, health maintenance, diet, family planning, and emotional well-being, while documenting progress.
  • Prescribes medications in accordance with state regulations, clinical protocols, and scope of practice.
  • Exercises clinical judgment to consult with or refer to supervising physicians as needed for complex cases, emergency care, or specialized treatment.
  • Performs office-based procedures and provides outpatient care for ambulatory patients within established protocols, available resources, and scope of expertise.
