About The Position

The Advanced Practice Provider provides patient centered, culturally sensitive, quality care using evidence-based medicine in accordance with hospital policies and protocols. Works in collaboration with the clinical team to provide patient and family education and support. Adheres to granted clinical privileges and collaborative agreement with the collaborating physicians, care protocols, HFSC policy on APPs, and medical staff bylaws, rules and regulations. Requirements

  • Master's Degree in Nursing with a defined clinical specialty.
  • Minimum of five years' experience in related clinical practice with demonstrated leadership and teaching skills.
  • Current licensure as an Advanced Practice Registered Nurse in the State of Connecticut and current certification in a specialty from the American Nursing Association.
  • Passed the State of Connecticut approved Pharmacology course required in Master's Degree Program.
  • DEA licensure required for narcotic prescription.
  • Basic and life support certification and advanced cardiac life support (ACLS training).

Responsibilities

  • Performs complete history and physical examinations upon admission, annually including pre-operative assessment.
  • Provides assessment, triage and therapeutic management of patients’ needs.
  • Requests appropriate consults as needed.
  • Orders and/or obtains all appropriate laboratory, radiological, and specialized tests to evaluate patients.
  • Implements care plans in collaboration with clinical team.
  • Consults with patient, family and, as needed, appropriately qualified physician on care plans.
  • Provides patient/family counseling and education.
  • Assists during procedures as privileged.
  • Provides accurate and timely documentation of all clinical interventions in a clear and objective manner.
  • Participates and identifies opportunities for improvement in patient care and outcome during daily practice, special projects and per review activities
  • Performs other duties as assigned as related to the APP role
