About The Position

Responsible for the provision of primary care medical services in a Patient-Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations. The provider will conduct examinations, review laboratory, referral, and imaging results to determine diagnoses, and document visits accurately using appropriate templates. The position requires accountability for attaining budgeted visits, meeting clinical care measures, and locking records within the 72-hour window. Additionally, the role is responsible for meeting Advanced Practice Provider professional focus and goals, providing comprehensive, continuous, and coordinated medical care for acute and chronic illnesses across all ages within a family unit in a community setting. This includes health promotion, maintenance, illness prevention, and applying a holistic healthcare model. Management of chronic diseases and acute illness with broad knowledge in procedural skills, coordination with specialists, and utilization of community resources is expected. The provider will advocate for access to healthcare, focusing on structured evaluation, early intervention, and health promotion, and will consult with supervising physicians or other team members when necessary. The role also involves promoting the Patient-Centered Health Home model of care and providing excellent customer service. This includes courteous interaction with patients, families, and staff, awareness of wait times, and providing patient education materials tailored to health literacy needs. Accountability for patient status as measured by clinical quality measures, participation in daily huddles, and incorporating ancillary health team members in patient care are key. The provider will also be responsible for setting and monitoring self-management plans, assisting in the assurance of clinical procedures, maintaining up-to-date clinical protocols, and completing chart and peer review responsibilities. Assisting the patient care team with performance improvement efforts, staff education, and FHCN promotion, including participation in rotations for medical, physician assistant students, and dental residents, is also part of the role. Participation in Network Health Fairs and other community events, as well as site and provider meetings, is expected. The position requires meeting Network and credentialing expectations for licensure and employment, adhering to the Attendance and Absenteeism Policy, and the ability to present to and work at any FHCN location based on business needs. Other duties as assigned will also be performed.

Requirements

  • Possesses specific advanced knowledge and skills, including written and verbal communication skills, computational, computer, and technical skills, and mathematical knowledge, frequently acquired through one of the following: Completion of an accredited Nurse Practitioner or Physician Assistant training program, with a minimum cumulative GPA of 2.5; or Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.
  • Performance of the duties and responsibilities of the job requires the equivalent of extensive formal training as a Nurse Practitioner or Physician Assistant, including an understanding of the application of the theory and practices of the profession to the operation of the organization as part of a Master’s degree (or other applicable extended training program).
  • Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
  • Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions, and other elements.
  • Licensed in the state of California as a Nurse Practitioner and/or licensed in the state of California as a Physician Assistant.
  • Requires employees to effectively communicate their professional opinions and extrapolations of information they collect, synthesize/analyze.
  • Employees must determine appropriate methods of communicating information through the use of tables, graphs, charts, and other visual forms.
  • Duties require the preparation and execution of presentations to large groups.
  • Regularly required to sit and use repetitive hand movements to type and grasp.
  • Frequently required to stand or walk.
  • Occasionally lift and/or move up to 20 pounds.

  • Responsible for adhering to the Attendance and Absenteeism Policy, recognizing that regular attendance is considered an essential function of all FHCN positions. Absenteeism is not being at work or failing to attend a paid workshop, training, or event unless the absence is protected by law.
  • Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.
